Exploration of expanded access of investigational products - Free Essay Example

Question 1 Should the public have a right to investigational treatment outside expanded access, defend your choice? Expanded access or the compassionate use refers to the outside usage of a clinical trial of an investigational medical product that is the medical product that has not been approved by the Food and Drug Administration FDA. Food and Drug Administration Expanded Access is legislation that aims at providing seriously ill patients who have exhausted the available treatments options and thus have no other comparable standard treatment option to gain access to biological agents and investigational drugs. FDA was enacted by the United States Congress to prohibit the sale of misbranded, mislabeled, or adulterated foods and drugs in interstate commerce. Ordinarily, investigational drugs may be used under controlled trial. Investigational medical products to be used by a patient as part of the clinical trial is preferable since the trial can generate crucial data that may lead to the approval of medical products; a good example is the case of Josh Hardy (Kroll, 2015). Patients with no ongoing clinical trials or are not eligible for any clinical trials can receive the medical product through expanded access when appropriate. The pharma and the FDA have in mind that the drug administered for trial and research purposes can benefit some patients, others experience no effect, and some patients can be harmed by the drug. The pharma and the physicians are faced with difficulties when prescribing drugs that are to be used for the first time. Some patients can refuse to take drugs which have not yet been approved as safe for use. Expanded access gives crucial data that help weigh the benefits and risks of the medical product. Question 3 Are the FDA regulations and guidance in place for expanded access sufficient in both protecting the public’s safety and also addressing the ethical concern of allowing patients with severe or life-threatening diseases the option to have access to an investigational product where the benefits and risks are still being explored? FDA has outlined set of regulations and guidance that govern on how medical products under investigation are being administered to patients with severe, life-threatening conditions. FDA has these regulations in place to ensure a balance between benefits and risk that can be associated with investigational drugs (â€Å"Food and Drug Administration,† 2016). According to the opinion of the author, the FDA regulations and guidance set in place for expanded access are sufficient in protecting the public safety and addressing the ethical concerns of patients suffering from severe, life-threatening diseases. For example, the Abigail Alliance once send a proposal to the FDA   suggesting that drugs should be made available to the patients with severe, life-threatening illness for purchase after the earliest testing stage. The FDA declined the proposal, one of the FDA regulations and guidance require three to four stages of medical product testing before it can be sold to the public ( Jacobson Parmet, 2007). The act of rejecting the proposal from Abigail Alliance shows that FDA is committed to ensuring the safety of the public. It is more dangerous to make medical products under testing and research available to the public in its initial stages; this poses more risks as compared to the benefits. Making a drug available to the public at its first stage of development poses a great risk to the public safety. The drug can have great negative impacts than positive impacts and thus harm a large group of people since it is not under controlled testing. FDA rejecting Abigail Alliance proposal proves that its regulations and guidance is sufficient to protect the public and patients with severe life threatening illness.

Children are interesting little humans. The prejudices and malice of society are hindered by the infant innocence, to such extent, that children do not comprehend what they say. Philosophy describes children as tabula rasa, meaning their minds are like blank slates. However, being blank slates can be a nuisance because children learn anything, despite the morality of what they are taught. The author, uses the characters from the story to portray the development of children’s mind and innocence. The girls’ attitudes change as the story progresses from ignorance, hatred, groupthink, to coming of age. Packer begins by describing the arrival of the white girls of Troop 909 to the Crescendo Brownie Camp, â€Å"They turtled out from their bus in pairs, their rolled up sleeping bags chromatized with Disney characters† (Packer 1). Some feel like it is the easy way out for people who have committed heinous acts, and others feel like it is the perfect justice for those individuals. While the Canterbury District Health Board as cited by Fineout-Overholt Johnston (2007), states that â€Å"evaluation is assessment of both processes and outcomes of a program or implementation. Irrespective of the fact that evaluation is the final step of Evidence Base Practice process, it should be an ongoing process, necessary all through the stages of EBP, to ensure that quality of plans and process. Evaluation of the evidence, implementation process and it’s effectiveness, before the dissemination to stakeholders, is an imperative. Hence, to ensure a comprehensive evaluation, certain factors have to be considered in the methodology and variables as it applies to the evidence sought in the project, it’s application and purpose, in order to yield the desired outcome for dissemination. These factors include; reasons for the evaluation, having a time frame, choosing the appropriate time to evaluate, who the evaluators will be and having an evaluation design or framework. It is also important to select an evaluation instrument, consider the data collection process, it’s analysis, manner in which results are reported, how they will be used and the costs of evaluation. In consideration of my choice of Evidence Base Practice project, which is using diet and exercise to control glycaemia and reduce complications in type 2 diabetes patients. The implementation logistics involved the use of strategic methods, such as health education of participants by nurses, dietitians, using power point presentations, healthcare model, leaflets, fliers, display of diet foods, measurements of weight and blood sugars, how to calculate food calories and the invitation of diet and exercise companies, to help motivate the patient’s participation in the project. â€Å"Consistent collection of data is crucial to a solidly documented program evaluation report, (Gard, Flannigan Cluskey, 2004). To achieve a reliable and valid evaluation of data collected, to generate evidence, a post structured questionnaire was developed in the following context: What is their attitude to health, wellbeing and the having to deal with type 2 diabetes, pre and post program. Effectiveness and usefulness of the methods adopted in the implementation process, Pre and post perception about diet and exercise, as a necessary intervention and it’s effectiveness in glycaemic control and prevention of diabetes complications. How motivated were they with the noted weight reduction, glycaemic control, provided incentives and cooperation received from diet and exercise companies and researchers. What role does culture, beliefs and socioeconomic factors, play in their choice of foods and it’s risk factors towards developing type 2 diabetes. Their overall impression of both positive and negative impact of the program. In addition, to evaluate the staffs participant experience, a meeting was organized in sessions to ensure everyone’s input, also using a separate questionnaire, asking the following questions: How helpful, was the methods and program aids used in the process. Pre and post participation rate, taking into consideration, gender as a factor. Adequacy of project funds, resources and it’s effect on implementation their whole impression, attitude, perception about rating of the EBP choice and project processes. In evaluating the above implementation methods employed, using above questionnaires, in a survey, patients attitude is expected to increase positively towards health and wellbeing, as reflected in their commitment to their scheduled appointments and care as opposed to their noncompliant attitude before the program. This in turn will encourage a positive staffs attitude, morale and perception of an evidence base practice process. Further evaluation of patients and staffs perception on cultural influence, adequacy of project , materials and resources, will reveal a positive change in patients cultural beliefs about food types and their effects on glycemic control, compared to previously held beliefs by the Black Americans and Hispanics participants.
